※確切時間以所辦通知為準第一學年結束前 Befor the end of 1st academic year	申請免實習 Apply for Internship Credits Waiver	1.免實習申請表 Application for Internship Waiver 點選下載/Click to Download Word /Odt 2.學分抵免申請表 Application for Credits Transfer 點選下載/Click to Download Word /Odt 3.成績單或學分證明 Transcript or Credits Certification(s) 4.相關工作經驗證明 Related Work Experience Certification(s)		入學前累計「相關」工作經驗一年以上者，得於入學後第一學年結束前，出具證明申請免實習，其所缺學分應以其他課程學分補足。 Students who have one-year work experience related before the school starts should offer working certificates for applying the internship credits waiver before the end of second semester.

	申請校外實習 Apply for Off-Campus Internship	✅學生 Students ✅實習單位 Internship Institution	以下契約書擇一使用： 1.國內實習計畫合約書(有薪) Domestic Internship Contract With Payment 點選下載/Click to Download Word /Odt 2.國內學習計畫合約書(無薪) Domestic Internship Contract Without Payment 點選下載/Click to Download Word /Odt 3.海外實習合作協議書(有薪) Overseas Internship Contract With Payment 點選下載/Click to Download Word /Odt 4.海外學習合作協議書(無薪) Overseas Internship Contract Without Payment 點選下載/Click to Download Word /Odt	💡契約書請準備一式兩份，並請實習公司用印。 The contract must be prepared in duplicate (2 originals) and both signed by the internship company. 💡請依需求(有薪/無薪)選擇。 Choose the internship contract depends on the demand of paid or non-paid. 💡請特別注意，若為有支薪實習企業，必須協助替實習學生加保勞工保險、健保及勞工退休金提撥；無支薪實習企業需替實習學生加保一百萬元以上之意外險。

期末考前教務處行事曆 (Academic Calendar)	校外實習期間 During the Off-Campus Internship	✅學生 Students ✅指導老師 Instructor	校外實習學生訪視輔導紀錄表 (每一實習單位一份) Visiting and Guiding Record for Off-Campus Internship 點選下載/Click to Download Word /Odt	指導老師須親訪，並附上師生合照。 The Faculty Advisor should visit the institution of internship, and take 2 pictiures with students.
	校外實習期間 During the Off-Campus Internship	✅學生 Students	實習日誌及週報 Student's Dairy and Weekly Report for Off-Campus Internship 點選下載/Click to Download Word /Odt	學生於實習期間每日應填寫實習日誌，每周(或每40小時)應撰寫實習周報，並經實習單位簽核後送交實習指導老師審閱，作為期末實習報告之附件。 Students should write dairy and weekly report (or per 40 hours), and mail it to the Faculty Advisor after approved by the Internship Supervisor. Both dairy and weekly report should be attached to the final internship report.

	校外實習結束 The End of Off-Campus Internship	✅學生 Students	1.校外實習報告格式說明 Instruction for Final Internship Report 點選下載/Click to Download Word /Odt 2.[113學年度] 學生實習問卷填寫SOP SOP for Questionnaire to Internship Students 點選填寫 Click Here ⚠️實習結束繳交報告前務必填寫 Must be done before the submission of the final internship report	請於實習結束後兩周內繳交期末實習報告的紙本及電子檔至所辦。實習報告內容須包含實習日誌及週報。 Submit both paper and electronic Final Internship Report to the department in 2 weeks after the terminaton of the internship. Internship dairy and weekly report should be attached to the final internship report.
